Quite often, preoperative Diagnostics is not enough to establish the diagnosis and select the scope of surgical intervention.in such cases, intraoperative rapid histological Diagnostics is the method of choice. <strong>Purpose</strong> of the study. To evaluate the informative value of intraoperative rapid histological examination in detecting oncopathology to determine the adequate volume of surgical intervention during operations on the thyroid and parathyroid glands.</p> <p><strong>Materials and methods</strong>. The analysis of 6140 operations for nodular pathology of the thyroid gland performed over 6 years (2015–2020) was carried out on the basis of the surgical department of the clinic State Institution «V. Danilevsky Institute for endocrine pathology problems National Academy of Medical sciences of Ukraine». The main statistical indicators of intraoperative rapid histological examination in detecting oncopathology are calculated, in particular, the sensitivity, specificity and diagnostic value of the method.</p> <p><strong>Results</strong>. The percentage of true positive results during intraoperative rapid histological examination separately by year ranged from 86.2–97.9 %. When assessing the sensitivity, specificity, prognostic value and accuracy of methods, it was found that the above indicators increased: sensitivity — from 88.4 to 98.7 %; specificity — from 95.3 to 99.3 %; prognostic value of a positive result-from 79.9 to 94.4 %; the predictive value of a negative result is from 94.4 to 98.7% and the accuracy is from 92.3 to 98.87 %, which indicates a fairly high efficiency of the above — mentioned study. The combination of parathyroid gland pathology with other thyroid diseases significantly complicates preoperative verification of the malignant process, but it is accurately verified by rapid histological examination in 100 % (according to our data, five cancers were verified for five operations for suspected parathyroid gland cancer), and invasion of surrounding tissues is detected in parathyroid gland cancer. In this regard, the importance of intraoperative rapid histological examination is significantly increased for choosing rational surgical tactics.</p> <p><strong>Conclusions</strong>.
