TY - JOUR AU - Коваль, Сергій AU - Снігурська, Ірина AU - Мисниченко, Ольга AU - Літвінова, Ольга PY - 2022/06/09 Y2 - 2024/03/29 TI - INSULIN RESISTANCE AND ITS RELATIONSHIPS WITH CARDIAC-METABOLIC FACTORS IN PATIENTS WITH ARTERIAL HYPERTENSION WITH OBESITY JF - Problems of Endocrine Pathology JA - PEP VL - 79 IS - 2 SE - DO - 10.21856/j-PEP.2022.2.05 UR - https://jpep.endocrinology.org.ua/index.php/1/article/view/2022-06-15 SP - 32-38 AB - <p><strong>The aim:</strong> to study the relationship of insulin resistance (IR) with cardio-metabolic factors in patients with arterial hypertension (AH) with abdominal obesity (AO).</p><p><strong>Materials and methods.</strong>The survey included 86 patients (50 men and 36 women) with essential  AH of 1-2-3 degrees (I-II stages) with AO of I-II degrees, aged 40 to 59 years. Patients were examined using standard clinical, laboratory and instrumental methods. IR was determined by calculating the HOMA index, and if the value of this index was &gt;2.77, the presence of  IR was diagnosed. Data analysis was carried out using statistical processing using the computer program SPSS 19.0. for Windows XP.     <strong>Results.</strong> It was found that the vast majority of AH patients with AO (72%) were diagnosed with IR.  Among AH patients with AO and IR, in comparison with patients with normal insulin sensitivity, not only a significantly greater severity of obesity and the frequency of metabolic disorders (prediabetes, combined dyslipidemia, and reduced levels of high-density lipoprotein cholesterol in the blood) were revealed, but also a more severe course of AH: a significantly higher frequency of grade 3 and stage II hypertension, as well as lesions of target organs: left ventricular hypertrophy, in particular, concentric hypertrophy, thickening of the intima-media complex of the common carotid arteries and stage IIIa chronic kidney disease.  <strong>    Conclusions.</strong>IR in AH patients with AO is a pathogenetically significant factor both in the development and progression of disorders of carbohydrate and lipid metabolism, and in AH.</p> ER -
