POSSIBILITIES OF USING THE QUESTIONNAIRE FOR THE EARLY DIAGNOSIS OF PRIMARY HYPERPARATHYROIDISM

Abstract

The aim of the study: to develop a questionnaire for opportunistic screening of primary hyperparathyroidism.

Materials and methods. A questionnaire for early diagnosis of the disease was developed and tested in the Department of Endocrine Surgery of the SI "V. Danilevsky Institute for Endocrine Pathology Problems of the NAMS of Ukraine". The study group consisted of 50 patients who underwent surgery during 2021-2023 with a diagnosis of primary hyperparathyroidism. The control group/comparison group consisted of 138 patients without primary hyperparathyroidism who sought medical help in connection with thyroid pathology at the clinical departments of the Institute.

Results. Among the complaints, patients with primary pathology of the parathyroid glands the following prevailed: chronic pain syndrome in the upper and lower limbs, the presence of degenerative-dystrophic changes in the spine, reduced growth, cognitive impairment, the presence of arterial hypertension, shortness of breath, and urolithiasis. It is noteworthy that a certain part of the control group has each of the considered complaints, which indicates the non-specificity of such symptoms, but the frequency of the presence of some factors was probably higher against the background of primary hyperparathyroidism. Such factors include damage to the bone system: chronic pain in the limbs, a decrease in height; pathology of salt metabolism: urolithiasis and gout; vascular complications: hypertension and coronary artery disease.

Conclusions. The symptoms of primary hyperparathyroidism manifestations in general are not specific only to this pathology, but more pathognomonic are clinical manifestations from the bone system and salt metabolism. The development and implementation of an organizational algorithm for the examination of individuals with the aim of early detection of pathology looks promising.
